Patient app
Patients log meals and protein, plus weight, waist and blood pressure. Glucose and ketone readings are optional, chosen per patient.
A GLP‑1 retention platform for clinics
Metabolic OS pairs a patient app with daily checks, alerts and a nutrition and strength protocol, so your team knows who is slipping before they quit.
Free 90-day pilot for three to five clinics. In Dr. Larson's own practice, 89% of patients complete the 12-week program.
The problem
Many patients stop around the two-month mark, after early side effects and before results feel settled. Each one takes future visits and refills with them.
A meaningful share of weight lost on GLP‑1 medicines can come from lean mass. Without enough protein, strength work and tracking, patients lose muscle they need to keep.
Most clinics can't review every patient between visits, so problems surface late or not at all.

How it works
Your prescriber keeps the medication plan. Metabolic OS handles what happens between visits.

Patients log meals and protein, plus weight, waist and blood pressure. Glucose and ketone readings are optional, chosen per patient.

Every patient is checked daily. Low protein, missed logging and high blood pressure are flagged for your team to follow up.

A structured nutrition, protein and strength protocol that runs alongside the prescriber's plan, with targets the clinician sets for each patient.

Each patient gets a weekly summary of logging, protein, weight and readings, with one thing to focus on next. It downloads as a PDF to bring to visits.
Who does what
Logs readings and meals in the app.
Checks every patient, every day.
Go to your staff, or to our coaches on the coaching tier.
Receives anything clinical.
Medication and clinical decisions always stay with the prescriber.
Two ways to run it
For clinics with their own coaching staff
For clinics that want it handled
Patient data
Your clinic and Metabolic OS sign a business associate agreement before the first patient enrolls.
Each clinic's patients live in their own database. Staff see only their own clinic's patients.
Any service that handles patient data, including the AI that reviews readings and gives patients feedback, works under its own business associate agreement. Meal photos and voice notes are never stored.
The pilot
No software fee on either tier for 90 days. Success is agreed up front, and you continue only if the gates clear.
Week 2
Patients are set up and logging.
Day 30
Patients are still logging past the first month.
Day 60
Flagged patients correct course within 5 days.
Ongoing
Every flag gets a response within 5 days.
Who built it
Metabolic OS was built by Dr. Chad Larson, founder of The Adapt Lab in Solana Beach, California. He has more than 20 years of clinical experience in metabolic health.
The app and protocol ran with his own patients before they were offered to any other clinic.
